Items to Gather Before Beginning Your Application
- A valid ID such as a driver’s license or passport (if using a passport, please email, fax, or text us a photo of the passport)
- Account statement from your current retirement plan (if moving funds from an IRA, or an employer plan such as a 401(k), pension, etc.)
- Beneficiary information including: social security number and address
- After electronically signing the online application, please print and sign the documents (including signature card and transfer form). Once signed, send back to Midland via fax, email or secure upload

Identity Verification Law
Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account. We require that you provide your name, date of birth, taxpayer ID, address and ID information. During the account opening process, we may ask for additional documentation such as a copy of your driver’s license or other identifying documents.
